Application information

We collect your name, email address, chosen category, professional or training details, connection to frontier medicine, submission time, and preferences for optional updates and public recognition. Organizations provide relevant company information and proposed representatives’ roles. Do not include patient information, Social Security numbers or sensitive personal records.

How we use it

The Society uses its private Google Form and member register to review eligibility, administer membership and maintain records. A response copy is configured to be emailed after submission. Optional Society news requires your choice; declining it does not affect eligibility. Application and membership administration messages remain necessary.

Payments and donations

Stripe processes membership payments and donations. The Society records payment identifiers, amount, status, date and membership term or donation designation for administration, receipts and accounting. Card information is entered directly into Stripe rather than the membership form. Donation receipts require a donor name and email. Membership dues and donations are recorded separately.

Recognition and access

Application responses and the member register are private. A public member or organization listing requires separate confirmation. Industry membership does not provide access to other members’ private information. Society records may be retained where needed for accounting, legal or operational obligations.

Corrections and choices

Email info@frontiermed.org to correct information, request removal, end membership or opt out of optional updates. Include the email used to apply. Records required for legal or financial purposes may need to be retained.

Service providers

Google Forms operates under Google’s privacy policy; Stripe’s payment pages operate under Stripe’s privacy policy. These services may use their own cookies and abuse-prevention tools. This website does not add advertising or analytics trackers to membership pages.
